About This Service
A Work Permit is a permit for foreigners to work, conduct business, or be employees in various professions in Thailand. It is required in conjunction with a visa; foreigners must apply for a business visa or a Non-Immigrant Visa B and then apply for a Work Permit to be able to work in Thailand. The application requires a medical certificate certifying the foreign worker's health. This medical certificate must be issued by a physician licensed by the Medical Council of Thailand.
The certificate must certify that the individual is not physically disabled to the point of being unable to perform their duties and does not exhibit any symptoms of the following seven diseases:
Leprosy in a contagious or socially unacceptable stage;
-Tuberculosis in a dangerous stage;
-Elephantiasis in a socially unacceptable stage;
-Drug addiction;
-Vaginal disease in stage 3 (with rashes);
-Chronic alcoholism;
-Mental illness. Or mental retardation.
Checkup List:
1. Medical history and physical examination by a physician.
2. Blood pressure, pulse, weight, and height measurement (Vital Signs).
2. Blood test for positive syphilis reaction / VDRL /
Medical certificate documents for Work permit include:

2. Advanced Pulmonary Tuberculosis
3. Drug Addiction
4. Elephantiasis (Chronic Alcoholism)
5. Syphilis (3rd Phase)
Note:
1. Includes physician's examination fee, nursing service fee, and hospital service fee.

Terms and Conditions for Service
1. Please make an appointment in advance via the Health Up application or by calling 1772.
2. Please refrain from eating and drinking for at least 8-12 hours before your health check-up (plain water is allowed).
3. Applicants must be 15 years of age or older.
4. This program is for Thai citizens or foreigners working in Thailand (Expat) or married to a Thai citizen only.
5. If purchased online, please present your e-coupon before your health check-up.
6. Health check-ups are available at the Health Promotion Center, 8th Floor, Building B, Phyathai 2 Hospital.
